Mécanismes d'adaptation du coeur aux surcharges hémodynamiques. hypertrophie cardiaque gauche et insuffisance cardiaque, métabolisme énergétique cardiaque.
Description des travaux
Pour compenser une surcharge, le coeur met en marche une série de mécanismes pour s'adapter à ce surcroît de travail. Une hypertension artérielle mal contrôlée, les sténose ou régurgitations valvulaires significatives, le coeur en post-infarctus ou plus positivement, celui de l'athlète de haut niveau sont différents types de surcharge associés à un remodelage compensatoire du myocarde. Le coeur cherchera à produire de façon plus efficace l'énergie nécessaire pour continuer à assurer sa fonction de pompe. Souvent, le muscle cardiaque devra gagner de la masse pour continuer à remplir son rôle correctement. On nomme ce processus l'hypertrophie cardiaque.
Notre équipe s'intéresse depuis plusieurs années aux adaptations cardiaques dans les cas d'une surcharge de volume sanguin à pomper. Nous avons remarqué que la dilatation du coeur causée par le surplus de volume s'accompagne par un remodelage du métabolisme énergétique favorisant à court terme le glucose. De plus, nous avons démontré que, paradoxalement, ce mécanisme de compensation à la surcharge a intérêt à être ralenti le plus possible pour aider le coeur à maintenir sa fonction.
Nous sommes à mieux caractériser les mécanismes prenant place dans l'hypertrophie cardiaque et à identifier de nouveaux intervenants dans ce processus.
